- Login to your account here
- Go to the Audit form builder tab
- Create a form by editing the options available for the form
- Scroll down and save the changes
- Copy the embed code and paste it to the desired page of your site
Recommendations:
- Add the embed code to the homepage or have it on its own page
- Make sure to add the form is a location on the site that is easily seen
